CRT Emudriver for W7 does not start at 15 kHz automatically after reboot. You must manually enable a 15 kHz mode from the OSD Arcade.
(Does this information still apply?)

Yes, it still applies. But now there's an even easier way of enabling 15 kHz provided you use an HD 5000+ card: EDID emulation with VMMaker.
